What game is that boss from?

Dragon Quest 4! Specifically the DS remake, Chapters of the Chosen.

The final boss’s gimmick is that (related to story-stuff, though I wouldn’t say it’s super-substantial, this is a remake of a NES game and all) he morphs through seven different forms, starting with the orange dude and then resulting in that green two-headed dude at the end (specifically orange, one-arm orange, no-arm orange, armless green, arms green, buffed legs green, two-headed green). It’s an endurance fight, which makes sense, given you go your way through the game with a max party of 8 characters.

For what it’s worth, I never beat him. :V This was during the time in my life where I mostly grinded my way through RPGs, though, so I bet I could do it now if I could find my game card.
