I'll start by saying I headcanon Gillen as aromantic and gay. Possibly on the asexual spectrum, but more on the grey or demi side of ace.
Why this matters is I tend to think of Gillen as someone who is quite kinky but in a self-contained way; I don't think he's especially interested in sex between himself and someone else, but he has a rich set of fantasies that he explores privately. And yes, during college, they definitely involved Tenma.
I feel like Gillen is on the more dominant side. Possibly because of some internalized homophobia about it, but he would prefer to take care of someone over being taken care of.
If he did decide to have sex with a partner, I think he would super be into role play. Like I said, rich fantasies. But also in part it's the fact that he's "not really himself" when he's with a partner at that point? Idk I just get this vibe that the idea of being seen intimately by others is extremely uncomfortable for him, so he can't be himself.
Not quite an NSFW headcanon but I think Gillen just has a hard time with being perceived. He visibly detests being judged, but like, by nature that is what humans do when they perceive. Between his sense of superiority and low self esteem one-two punch it just makes him not want to have a body. So when it comes to sex and sensuality he would really rather keep that to himself and not involve a partner. He could be convinced I think, and it would be healthy for him to learn to be appreciated, but he definitely isn't there inherently.
I know there's a joke in the fandom about his recorder habit being brought into the bedroom, but I honestly don't really think that would come to him inherently. To Gillen, the tapes are to keep permanent exact record of what his patients have said, and used as pieces of evidence; now if a partner asked about recording them then he might find that interesting, but yeah, I don't think it would come to mind.
He's definitely capable of matching someone's freak. Like, Lunge? That level of freak? He can match that. He's one of those guys from the posts that are like "when they have sex they're both standing on opposite sides of the room fully clothed engaging in psychological warfare". Try stimulating his mind he might like that.
