How had Jefferson managed to die before him?

He was the sick one. He's always ill. But Jefferson died before him, somehow. How is that possible? He'd believed he was going to die years ago. Every single time it was hard to breathe after a coughing fit, he'd wondered if he would recover that time.

But Jefferson? He's been healthy for his whole life.

It didn't make any sense.
