#facial weakness

Hi friends, quick question. Anyone had facial weakness with migraines / headache? I CAN still smile normally if I try but basically my face feels weaker / droopier on one side. Second time it's happened recently. Can it just be teeth clenching causing migraines, causing this? Or maybe the clenching causing inflammation in turn pressing on facial nerves? No aura (none of my migraines as an adult have had aura), no confusion or slurred speech. Just an awful ache and my face is weakened and slightly drooping on one side.

Hi guys! Just got back from the dentist. so no talking this time. Transcript: (the youtube captions on the video might be slightly more accurate. this is the script I made before I filmed) Hi Guys! Sorry it has been a long time, when we moved, I lost my webcam! I wanted to give an update of how I have been. I had surgery on my eyelid a few weeks ago and had a weight put in the lid to help it shut. Also, the face paralysis is better I wanted to show how I am doing. For the next one, I will hold so the the movements not being pulled over by the stronger muscles  I cannot hold air in my cheeks yet. the corner doesn't close. My GET is better, but it's not gone. When I look up or down, there is no sound, looking this way makes a quieter sound, but this side (right) is still loud, but not as bad. Maybe because of my nystagmus - my eye shakes a bit, so that maybe why. I'm very happy with my recovery and I will keep working. I will get better. Bye!

Surgery was Monday afternoon. it was super easy. it only took a couple hours, and it barely hurts at all. I can hardly believe how fast the bruise is fading. My  left eye was unable to shut due to facial weakness that I got after I had an acoustic neuroma removed. I had a surgery called ‘upper eyelid gold eyelid weight insertion’. Immediately after surgery I was able to close my eye. Then the swelling and bruising set in. I Highly recomend this surgery to anyone who is unable to close their eye fully for any reason. It really evened out my eyes when they are open and lets me cut back on the eye drops that I need during the day

I still have some swelling, surgery was only 4 days ago! but I am still very pleased with the closure of the lid that I am able to do!

Kitten here. My surgery went very well. after one day, the swelling is down a ton, and I just have a yellowish bruise!

Bell's Palsy - Idiopathic facial nerve palsy

abrupt onset, typically overnight of unilateral facial weakness

ipsilateral pain around ear + reduced taste (chorda tympani branch) sometimes present

NB - forehead involvement (unlike stroke = UMN forehead sparing)

rule out other causes in atypical presentation e.g. meningitis, malignancy (parotid, brain stem, acoustic neuroma), MS, DM

must exclude Ramsay-Hunt syndrome (reactivation of varicella-zoster --> vesicular rash in auditory canal)

Management - <72hrs give prednisolone (60mg, reducing dose for 5d), ?benefit from aciclovir, protect eye if unable to close

Prognosis - complete paralysis up to 80% make full recovery, 15% delayed may be complicated by aberrant reconnections
