Kaura tossed her things down wearily in the cave the Brotherhood was currently inhabiting. Most of the men were still outside, celebrating their recent victory over a large Lannister party.
For that reason, she was unaware of Fandral’s presence just around the bend, and as she rounded that bend, she came upon him in a most confusing state. He was wearing only his blue cape and underclothes (which were notably too small for him and thus made certain bulges much more noticeable).
At first Kaura thought to turn tail and retreat, but the urge passed in an uncharacteristically bold train of thought. “Well, well, my friend,” she chuckled, leaning against the stone wall with her arms crossed over her chest. “This is indeed a new look for you. It is one I do not think I mind.”

I have a thought that Drogo came into the marriage looking for a wife that would be his equal and not just a pretty face to look at or someone to own as a possession. If he wanted something like that he would have taken a dothraki wife and spared himself the trouble of searching for a woman who was different in every way. The Khal wanted to feel something more than what he had grown up seeing with his father and the other Khal's that they met within the city of Dosh Khaleen.
Now after their first night together and seeing how meek she was he probably banished the thought of that ever happening, and started to treat her as he would a dothraki woman - since that was how she was acting (can't really blame her for that... ).
The moment that it really changed and the dead thought became a quick reality? That was definitely when Dany took him under the stars in front of the khalasar and showed him that she could take control of him just as easy as he could while they were intimate. She showed him that she wasn't the meek little girl, that she was the woman he wanted from the very beginning. The idea grew stronger and probably became common knowledge within the Khalasar when Dany figured out she was pregnant with Rhaego.
So.... Short answers? Between the night they made love under the stars and them finding out she was with child is when I think they became equals.
