How about an AU where Nao Shimura survives Tenko's quirk manifestation rampage.

1- Nao packs a bag after her conversation with Kotaro. She isn't sure if he's going to make things right, she hopes so after their talk, but she has to be sure either way. She's packing a bag for the kids when she hears the screams outside. By the time she gets out there, Mon, Hana, Kotaro, and her parents are gone, and Tenko is sitting in the wreckage.
2- He scrambles up to meet her, crying, but as his hand brushes a broken piece of tile, it turns to dust. Nao orders him to stop for a second, let her figure out whats happening. She goes to the house and grabs a pair of gloves, telling him that if he wears them, everything will stop breaking. It works because he thinks it'll work, and she holds him while they both cry.
3- When Nao is out of tears and Tenko has passed out, she picks him up and goes through the house, getting all the money, the photos, the valuables. Then she gets a cab and goes to a hotel, tucks Tenko in, and spends the night researching on her phone what she should do, what heroes would do, what would happen to her son. It terrifies her. She can't even pretend some villain did this, it'd be discovered as soon as she registered Tenko's quirk.
4- She hasn't worked since before she got married, but she finds a job in a restaurant and a room to rent above it, the manager agreeing after she tells that her husband and family were just killed in a villain fight, and she's still waiting for any insurance pay, isn't sure if it'll come from the hero but she's got to feed her surviving son. It's peaceful for a few weeks, but then she realizes this resturant is likely a front, and when one of the men collecting some 'payment' from her manager scares Tenko and he accidentally dusts a napkin holder its confirmed in the form of the right hand of the yakuza head knocking on her door that night, promising higher pay and protection in return for Tenko eventually becoming a member. Nao, seeing no way out, quietly agrees and hopes her son will forgive her.
5- A few months later, a way out appears- a white haired man with red eyes slaughters through the yakuza, ending with the collectors in the middle of the restaurant, the manager and Nao watching in terror. He tells her he's an old family friend of the Shimuras- of Kotaro's mother- and has been looking for any survivors after hearing what happened. He's something of a quirk expert, and he can offer better safety, if Nao and Tenko come with him.
