Hugo Weasley has always been like Rose's little cub. Growing up Hugo was so, so important to her. She felt like it was part of her duty to take care of him, to make him grow into something beautiful and pure. She's so happy that he grew up to be who he is now, and she can't even take credit for it because he was born to be amazing, born to be good. When she entered Hogwarts and was sorted into Slytherin, she felt a bit guilty. She knew then and there that there would be a divide between them, and she's always felt like she abandoned him.
He's the only person she would give up everything for. She would give up her ambitions, her life, and her dignity--would lay it all down in a heartbeat if he asked--and that makes him her complete undoing. There is quite literally no one in this world she feels more connected to on an emotional level. Even though she's had to push him away, she hopes that he feels that connection because she knows it's there. She hopes more than anything that he won't take their separation and make it into something that's his doing (though she knows he will, because he's too much like dad.)
Eventually she hopes that she can take care of him again, that she can make sure he feels good enough. It kills her to see him all the time and ignore him. It's like the worst kind of torture of the soul (which makes her feel better because she should suffer for what she's doing, what she's putting everyone through.) When he was born her world shifted and when he said her name (though she can't remember it, she was too young) her world switched from revolving around herself to orbiting around him. Without him she feels empty and useless.
