The Hunter alphabet just replaces Japanese letters (aka kana), so everything that’s written in ‘Hunterese’ can be easily deciphered back to Japanese. All the characters also, obviously, speak in spoken Japanese. However, imo, i think it may be ok to assume that Hunterese is actually a completely different language that is just portrayed as Japanese because making up a whole language is hard.
Japanese in it’s full form does exist in Hunter world’s Japan (where Hanzo, Nobunaga, and Basho are from), and that’s why Basho’s haiku are written in Japanese.
Chinese also seems to exist in the Hunter world; It’s Feitan’s first language and he speaks it when he’s angry. Feitan is also portrayed as having a heavy accent in the manga/anime.
Kurapika’s tribe (the Kurtas) spoke an entirely different language from everyone else; he had to teach himself the main language and, imo, that’s probably why he speaks so formally/he speaks the textbook language. (Like he uses the formal “Watashi” when speaking aloud but he uses the informal “Ore” in his own thoughts, so he probably still thinks in his own language). Also, everyone in the tribe speaks in Japanese in the manga/anime because, y’know, the Japanese audience knows Japanese, but the Kurta language is stated to be a completely different spoken and written language.
But the ‘Hunterese’ language does definitely seem to be the main language, to an almost unbelievable extent. I dunno if there was some worldwide initiative/government crackdown for Hunterese to become the main language throughout the whole of the known HxH world, but for even the completely isolated NGL to speak it seems quite odd. (Japan, in the hunter world, is considered a very tiny, mostly unknown country, so that may be why they have their own language. The Kurta clan, too, completely isolated themselves from the outside world.)
So, uh, (tl;dr) in conclusion:
'Hunterese' is spoken Japanese with a writing system that's just a simple substitution cipher for Japanese letters/kana. However, considering that Japanese in it's full form is used exclusively by the people of Hunter Japan, I think Hunterese could be interpreted as a completely different language that is just portrayed as Japanese for the sake of convenience.
Other languages exist (Japanese, Chinese, Kurta language), but they seem to be extremely rare.
