My one of my most "American out-of-place in Japan" moments was when I expected my monthly health insurance cost to increase after having a kid. It took MONTHS for me to finally realize something was different in Japan, and turns out for the most part, heath insurance is a set percentage of someone's income - usually around 5%.
Turns out it is not inherently normal to start billing people more for healthcare just because there's someone new in their family.
