#knitting trouble

Call for Pattern Help! (Knitting)

I am making the medium size of the below sleeve pattern, which is the first number INSIDE the parentheses.

I have cast on 41 stitches, and after the final increase I should have 65 stitches. However, the math ain't mathin'. As I read this pattern, it should be 41 + 1 + 11... which only totals 53.

Can someone tell me where the extra 12 stitches are supposed to come from?

I'm terrible at remembering stitches. It's knit 2, purl 2, brain - not rocket science. I miss doing full rows of one or the other.
