Mandibular setback with chin correction This young woman had always had a large lower jaw. Her mandible had always been protruding with a slight but progressive asymmetry towards the left side. There was also an anterior crossbite, which made biting difficult for her. She also had difficulty with uttering certain consonants because of this. The patient also had a very prominent and masculine chin. Upon finishing her studies, she and her parents decided to get this corrected. They were also worried about the progressive asymmetry. They approached a dental surgeon in their hometown and was referred to us for treatment. Dr SM Balaji, facial asymmetry correction specialist, examined her and obtained imaging studies. He explained to her that she needed fixed orthodontic treatment for fine alignment of her teeth before surgery. This would be followed by bilateral sagittal split osteotomy for mandibular setback with midline correction. Final stage would be a reduction genioplasty of about 6 mm. The patient was in agreement with the treatment plan and consented to surgery. Treatment commenced with six months of fixed orthodontics. Once satisfactory teeth alignment had been achieved, the patient was prepared for surgery. Under general anesthesia, bilateral mandibular retromolar incisions were made and bone cuts were made using a saw. Care was taken to ensure that there was no damage to the inferior alveolar nerve. Mandibular reduction of 8 mm on the right side and 5 mm on the left side was performed to correct the asymmetry. The nerve was localized in the proximal segment allowing for manipulation of the distal segment. Occlusion was checked and the mandibular segments were stabilized bilaterally with plates and screws. Attention was then turned to the prominent chin. A reduction genioplasty was performed with removal of a 6 mm wide segment of bone from the chin.
