Inosuke was, originally, a human child. He's always smiling and cheerful, even in the face of a terrible destiny.... he's ill and dying. He's slowly being turned into a 'roid to prolong his life, piece by painful piece, but there's no hope for a cure.
"Monitsu" only wants to keep that precious smile safe forever, but he's terrified. What if they don't make it in time, and Inosuke dies? What if they do make him a complete mechanical body, but once he's turned into a droid Inosuke loses himself and becomes someone completely different?!?!?!?! What if he can't feel things with his skin? What if he can't feel things with his heart and soul? What if he won't like Monitsu anymore, or smile at him ever again? So, Monitsu swears that once he grows up he'll find a way to build Inosuke a body that's so advanced, so incredibly human, that Inosuke will manage to remain the same, and still love Monitsu as he does now, so innocently and deeply.
