Did surgical cric on a pig trachea today, my first AC IV stick, successfully intubated 3 of the adult dummies, whereas before I could only get the ped., learned the needle decompression skill AND passed my megacode testing. Today was a fun and productive day, and totally worth spending my Saturday at school.

Risk of medial vascular structures with needle decompression in 2nd intercostal space with mid-clavicular approach

GO LATERAL: Do needle decompression in 4th/5th intercostal space in mid-axillary line (CHEST TUBE PLACEMENT SITE). 

Use 18g angiocatheter (in central line kit)

In EMERGENCY situation FINGER THORACOSTOMY will allow for quick decompression and will provide incision for subsequent chest tube.

Can then place chest tube in more sterile fashion (if patient stable).

BILATERAL FINGER THORACOSTOMY for diagnostic purposes in crashing trauma patient.

There's this person in my head.

He's brilliant. Capable. He can do needle decompressions and cricothyrotomies, he can run a code without freaking out. He's a really good paramedic. Maybe even a great paramedic. He's me. Only so much better.
