If you ignore everything about ATB, Dr. Tenma's dislike of Ochanomizu and his ideals in the "Once Upon A Time Tales" is actually really petty, because it seems like they barely know each other but are still at odds. And their conflict seems pretty one-sided. Tenma talks shit about Ochanomizu for not being interested in power and politics while being well liked as a veteran scientist and Ochanomizu finds being called Tenma's "enemy" just ridiculous. There's no complicated backstory and they never personally engage with one another throughout the entire story.

It's just one prideful asshole hating on an idealist who is a better person than him. That's all there is to it.

And I personally find that way more fitting for Tenma's character than being a former friend. Of course he would have issues with a man he barely knows, he's just that full of himself.

Side note: I kind of like that there is a subversion in the "elder vs new generation" trope when it comes to Ochanomizu and Tenma. Despite having being several years of life and work experience over Tenma, Ochanomizu is not portrayed as hubristic over his position as the head of the Ministry of Science as Tenma was, who is typically portrayed as power hungry and over achieving.
