As mentioned briefly in Alex's backstory, Alex struggles with depression (or something similar). While he usually manages to keep his thoughts at bay with his job or otherwise, there are times where he resorts to drinking to go numb and block them out.
When people approach him in bars, he entertains them for a bit but never bothers to grow close to them. He knows that they're usually there for one thing: sex. He'll give it to them, sure, but he never intends to get any more intimate. While he enjoys it, he really doesn't care much about sex as an act of close, romantic intimacy.
To him, sex is simply another thing to pass the time, just like drinking. It's an easy thing to keep his mind on and distract himself for a while, and it's much more interesting than just sitting at a bar (or at home) drinking his sorrows away. People believe this is completely selfish of him-- and it is. He's simply trying to put himself first after years of neglecting to do so, and he doesn't really care about the way he's going to do it.
He knows that he should seek help instead of bottling up, but years of pushing his own mental health to the side in favor of his previous line of work has resulted in his complacence. He's grown used to his struggles not being taken seriously or brushed to the side-- after all, everyone suffers. He's no different than them, right?
