A malignant bone process is more likely to be 1 or more metastasis than a primary bone cancer, and more bone mets are (osteo)lytic than osteoblastic (or sclerotic). Lytic bone lesions arise when tumor cells from a primary tumor in a different part of the body are deposited in the vascular beds of the bone matrix and grow with the help of parathyroid hormone-related peptide (PTHrP) stimulating osteoclasts to chew up existing bone. Sclerotic bone mets lead to more bone deposition through an uncertain mechanism. Both are seen better on CT or MRI than on plain xrays; sclerotic lesions are seen better than lytic ones on nuclear medicine scans (bone scintigraphy or PET-CT).
Pro-Tip: A single primary malignancy can lead to mixed lytic and sclerotic lesions.

Ch 1 - First Last Day (excerpt)
Excerpt from my ONC 2026 book, To All the Óskars. Find it on Wattpad!
We both entered the class as the bell rang. Dr. Archer’s classroom was as plain as his clothes. The walls were a blank beige, minus a few outdated meme posters. The desks were arranged in pairs. The chalkboard had the words “Multimedia 101” written at the top. Dr. Archer stood at the front of the class. I sat in the middle, and the seat next to mine was empty. He watched as everyone got settled and stopped talking. “Good morning, class,” he spoke with a gravelly voice, “before we officially start, everyone will be in pairs for the semester. If you came early, please move back to your assigned seat with your partner.”
AmberJack was with Joshua. Kayla with Brandon. Justin with Simon.
“And finally, Edin Sørensen with Óskar Grímsson.” Mr. Thorn finished. “Seat yourselves and get friendly with your partner.”
“God be with you, Edin.” AJ smiled. She got up and went to join Joshua at the front of the class. I watched from my seat as people shuffled around the classroom. Justin, who was sitting near me, got up to find his partner. Kayla continued groaning and complaining as Brandon sat down near her. Justin gave Simon a fist bump as he got close.
I started to wonder who my partner was. This Óskar. Was he a jock like Justin? A nerdy guy like Brandon? Perhaps he was mysterious. Maybe he was super smart like AmberJack. He also could love fish like me!
My train of thought, along with the conversations in the classroom, died as the door creaked open.
“Ah, there you are. I started to wonder why you were late, Mr. Grímsson.” Mr. Thorn said. “Your partner is Edin Søresen. Third row, middle.” The teacher instructed.
As I looked at Óskar, I just knew that my garden was going to earn more flowers this year.
