Oxyracetam (substance development code ISF 2522) is a nootropic drug from the racetam family and a very mild stimulant. Several studies suggest that the substance is safe even when large doses are consumed over long periods of time. However, the mechanism of action of the racetam family of drugs is still under investigation. Oxiracetam is not approved by the Food and Drug Administration for any medical use in the United States.
Efforts have been made to explore the possibility of using oxyracetam as a drug to alleviate symptoms of dementia. [6] However, no convincing results have been obtained from studies of patients with Alzheimer's dementia or organic solvent abuse.[6]
Tests conducted on patients with mild to moderate dementia showed beneficial effects as measured by higher scores on tests of logical ability, attention, concentration, memory and spatial orientation. Improvements were also observed in patients with exogenous aftershock syndrome, organic brain syndromes and other dementias.
DBA mice administered oxyracetam showed a significant increase in spatial learning ability, as determined using the Morris water navigation task, compared to the control group. This increase in yield was correlated with an increase in membrane-bound PKC.
Pharmacokinetics Oxyracetam is well absorbed from the gastrointestinal tract with a bioavailability of 56-82%. Maximum serum concentrations are reached within one to three hours after a single oral dose of 800 mg or 2,000 mg, with maximum serum concentrations reaching 19 to 31 μg/ml at these doses.
Oxyracetam is excreted mainly by the kidneys and about 84% is excreted unchanged in the urine. The half-life of oxyracetam in healthy subjects is about 8 hours, while in patients with renal impairment it is 10-68 hours. There is some penetration of the blood-brain barrier, with concentrations in the brain reaching 5.3% of blood concentrations (measured one hour after a single intravenous dose of 2000 mg).
The clearance rates range from 9 to 95 ml/min, and steady-state concentrations after administration of 800 mg twice daily range from 60 μM to 530 μM.
The highest concentrations of oxyracetam in the brain are found in the transparent septum, followed by the hippocampus, cerebral cortex, and lowest in the striatum after an oral dose of 200 mg/kg in rats.[6] Oxyracetam can be quantified in plasma, serum or urine by liquid chromatography using one of several different detection techniques.
The main metabolites of oxyracetam include beta-hydroxy-2-pyrrolidone, N-aminoacetyl-GABOB, GABOB (beta-hydroxy-GABA) and glycine. Phenylpyracetam and all other members of the -racetam family, as well as pyroglutamic acid.
