Every narrative point I've read about the circumstances of his demise points to the fact that some part of Sanguinius' soul is still stuck in the eternal instant of his death. Pieces of him roam free, powered by the will of what he represents to the Imperium and to his Sons - such as the Sanguinor - but a core part of him has been left behind in the clutches of the Great Four, a nexus point of eternal torment.
The Blood Angels' curse of the Black Rage makes them constantly live through his agony, the Rage powered by his eternal torment. His death throes are eternal and undying - his flesh may be dead, but his soul is immortal, and it is a point of deep, visceral horror.
Perceus was Sanguinius' very first friend, his very first human contact as a child, and later on his very first Blood Angel that he had taken to the stars when the Emperor of Mankind had found him. The psychic ripples of his death had pushed her over the thin boundary of the Materium and the Immaterium, straight into the realm of nightmares spawned by Sanguinius' tormented mindscape.
For ten thousand years she wandered this nightmare realm, looking for her Lord and her dearest friend. This is the moment where she finally finds him, wrung out and tormented beyond belief, delirious from the simple pleasure of seeing his trusted friend.
