Tourou no Ori is a wonderful historical yaoi manga. The art is beautiful and the story is super tragic. It shows the trials and tribulations an heir might face in the Showa era. The simple and very salacious existence of two brothers, a loyal manservant and their twisted relationships with each other and the outside world.
TOP: (picture not seme!) Ranzou
Ranzo is the unfortunate first born in the Touma family. He is mute and barely above an animals intelligence. He is the "real" (secret) heir.
Bottom: ( this may work both ways ;) Ikurou
Ikurou is the second born but acts as the true heir since no one outside of the family is aware of Ranzo being the true first born. Ikurou has to be the face of the Touma family and deal with all of its affairs. He only has one perk that I can think of....

He is to show the young master "the ropes" (among other things)
He is sexy, suave, well mannered and reliable. Everything you could want in a dramatic master/servant relationship.
