I think I can help you out!

First of all, Chibi Chibi is totally unrelated to Chibiusa. Her origins differ greatly in the anime and manga (you can read about it here) but she and Chibiusa are two completely separate people.

Chibiusa is the daughter of Usagi and Mamoru. She comes from the 30th Century.

Black Lady is the corrupted, evil form of Chibiusa. Chibiusa's biggest dream is to grow up and become a lady like her mother, Neo-Queen Serenity; Wiseman (the main villain of the second arc) twisted her and her dream into something evil and used his powers to turn her into Black Lady, a grown-up version of Chibiusa.

Princess Lady Serenity is Chibiusa's real adult form. She never actually appears in person, only as a vision.

And even though you didn't include Small Lady, I'll explain her anyway. She's the Princess form of Chibiusa (like Princess Serenity is Usagi's royal form).

So basically, Black Lady, Small Lady, and Princess Lady Serenity are just variant forms of Chibiusa.
