Person A being injured and unable to care for themselves, but they are the only competent person when it comes to treating wounds. So they have to instruct person B on how to take care of them.
Bonus points if the treatment is super painful and there are no painkillers, or there are plenty of painkillers available but person A refuses them bc they have to keep their head clear.
Props to you if A says: "no matter what comes out of my mouth, keep going" And when they scream, they scream whumper's name or a broken "please". Perhaps they say "thank you" and person B thinks it's for treating them, but if person A didn't say it whumper would hurt them so much worse.
