Resurgery of Malaligned Subcondylar Fracture of the Mandible This young man had been involved in a motor vehicle accident approximately a month ago and had suffered a left subcondylar fracture of the mandible. He had been operated in a bigger hospital, but the fracture reduction had been mismanaged with poor reduction of the fractured segments and resultant malocclusion. There was also an unsightly scar from the extraoral approach adopted to perform the surgery. He had problems with eating subsequently and frequently heard grinding noises from the fracture site. A local surgeon had then referred him to our hospital for corrective surgery. Imaging studies obtained at our hospital revealed a poorly reduced fracture with a gap between the fracture segments. Examination revealed that he had malocclusion. The fracture site was accessed through the old scar and the plates were removed. If the patient had presented to us at the time of the fracture, we would have accessed the fracture site intraorally to avoid scar formation (using an internal fixing system). The fracture segments were correctly aligned and occlusion was checked. It was then stabilized with plates and screws.
