It involves blood - Elena likes to the way Sam tastes. Actually, I was going down this road but I don't really feel much kink/dark stuff for them. I suppose I get enough of it in their canon relationships.
Elena has to use her magical super vampire powers to reach up high enough for the kiss. And she likes him on top because it makes her feel small and human but he's gentle so she doesn't feel trapped or vulnerable. Sam likes her tiny feet at the small of his back.
