How is Don double mutated? I've been scrolling for a while but haven't been able to find out how

It'll take a LOT of yapping about my universe to get the full story so I'll give you a tldr: a universal rift is torn and mutated beasts from that realm come through, not too many but enough. If they cut you or get any of their mutation juices on you (they're basically just coated in it because of the world they exist in), it causes a mutation.

It's heavily based off the second mutation arc in 2003! I hope this clears it up a bit :) I'll try to post more about this kind of thing

OOH! its it cause of Donnie’s second mutation that caused his height and built-maybe there were some side effects of his mutation that follow him later on?

No, Don's just a big boy!

There are definitely lasting effects of his second mutation though. Easier to anger for a while, doesn't like being touched (not that he cared too much for it before), always at risk of the second mutation being triggered again... fun things.
