He was forced to kill his brother hundreds of times, at first Insans started by stopping looking into his eyes, then he avoided him as much as possible. When he went insane, he killed his Papyrus by himself, without being controlled. He did it for a reason, but it was something cruel anyway.
He's afraid that his brother (which he sees in any Papyrus) may tell him that he hates him, he's afraid to look at his eyes and see tears. His insanity has led him to obsess over it to a point where it has become a phobia. He can't see a Papyrus without freaking out.
