I found this awhile back on a CDC page called “Smallpox Risk Evaluation Help” and I saved a pdf of the original page on my hard drive. Unfortunately, the area of the website where I originally found it has been altered considerably since I was last there, so somewhere amongst all those links are these four images. Anyway, I made them into a nice chart using the original text.
