Requirements for Smart SIM Registration
Active SIM card: Ensure your SIM card is working and has network access.
Valid ID: A government-issued ID such as:
Philippine National ID
Driver’s License
Passport
SSS/GSIS ID
Senior Citizen ID
Voter's ID
UMID, among others.
Personal information:
Full name
Date of birth
Address
Gender (optional)
Selfie photo: For identity verification.
How to Register Your Smart SIM
Online Registration
Visit the Smart SIM Registration Portal: SMART SIM Registration
Enter your mobile number and click Send OTP.
Input the OTP (One-Time Password) sent to your mobile phone.
Fill in the required details and upload a photo of your valid ID and a selfie.
Submit the form and wait for confirmation.
