Somnote brand chloral hydrate has been discontinued in the United States.  Liquid chloral hydrate was discontinued several months ago.  This leaves no chloral hydrate product on the market for American patients.  As a result it is necessary to carefully taper off the medication while supplies last.  My transition is planned as follows (only sleep medications listed):

Pre-transition:

-  1mg Ativan

-  50mg chlorpromazine

-  1000mg chloral hydrate

Transition

- 1mg Ativan

-  50-100mg chlorpromazine

-  50mg hydroxyzine

-  100mg gabapentin

-  500mg chloral hydrate

Post-transition

-  1mg Ativan

-  100mg chlorpromazine

-  50mg hydroxyzine

-  100mg gabapentin

The gabapentin (Neurontin) was added in for additional sedation and a negligible antiepileptic effect, chlorpromazine (Thorazine) was increased for sedation, and hydroxyzine (Vistaril), previously 3 times daily as needed, now specifically has a dose at night.  This will be very difficult and I wrote this to show my process because it will be even harder for some than it is for me.
