Why are Layer 7c (effects changing P/T) and Layer 7d (counters changing P/T) not one layer (stuff doing math to P/T)? Is there a specific interaction that's ambiguous if applied in the other order?

There’s currently no reason for 7c and 7d to be separate sub-layers other than history. Before Magic 2010 changed a bunch of the rules, there were only four sublayers of layer 7, with what are now b and c as a single sublayer. 

Yes, that was as terrible as it sounds. It meant that you could, for example, have your Mutavault get -1/-1 from an opponent’s Peppersmoke, then activate your Mutavault’s animation ability again to make it 2/2 again.

When this was the way of things, it made sense to apply counters after the other effects so that an animated Mutavault with a +1/+1 counter on it would be 3/3, for example.
