So, when Castiel went down to save Dean Winchester, he was an angel in hell. He was like a beacon, and there was no way in hell (hehe i makes puns) that the demons didnt fight him. Cas fought back all the demons, through hell fire, which was burning his feathers and exhausting him, all the while looking for Dean Winchester.

And Dean definitely wasn't just in a nice cell by the front door, he was one of Hell's most prized. He was somewhere in the lower dungeons, hidden away so that he doesn't escape, nor is helped in escape.

And then, because Cas is an angel, he cant take Dean away unless Dean wants to come, so he had to tell Dean Winchester-- who was lost, alone, beaten to pulp and tortured-- who he was, and that this was real and it wasnt a mirage. And then he held him with one arm, all the way back out of hell and out, protecting Dean and himself from the demons who were definitely now opposing harder than before.
