Transition care program is designed for helping older people getting back on their feet after staying at hospital, providing short-term care that includes nursing support, social work, allied health care and personal care.
The importance of this program
Older people usually prefer staying in their own house and this program helps avoiding the necessity for long-term care, delaying shifting to old age home for as long as possible. After a hospital stay, they can get back to their feet with the help of transition care program.
