He recognizes the serpent immediately as one from Ryuuchi Cave. Realizing what it's doing here with its body wrapped around pieces of a sword and a ribbon around its neck takes a moment longer. At first, Sasuke thinks this is a gift from Aoda; unusual, and also Aoda shouldn't know his birthday, but this is still his first thought.
Unthreatened, he reaches down and takes the pommel and hand guard with an appreciative murmur. Also, it'll relieve the small snake of the heavy weight. It's then the snake reveals Orochimaru is the one who, presumably, the gift is from.
Orochimaru didn't come to wish him happy birthday himself. Sasuke doesn't mind, but it is noticed nonetheless.
