A couple of things here: 

1) I really like Dr. Hiliruk’s placement on this cover. For one, he was the original bearer of this flag and the forerunner of both Chopper and Luffy’s convictions, so having him stand over them both is really fitting

2) Both Hiliruk’s hair and the buttons of his shirt are in the shape of a cross, which in the Western world at least is one of the universal signs for healthcare. 

3) Hiliruk’s color scheme is hideous and I kind of love it.
