Now that I've woken up from this disaster with actual nightmares I want to point out once again that dean had this messy room with empty pizza boxes and beer all over because he just couldn't take Cas's death and went mourning all over again, trying to repress his feelings by eating, coping mechanisms we've seen so too many times and he just let that rusty nail take over because he couldn't do it anymore, live without cas. So I'd like to think, even if it's outright denial and make-believe, that dean had the roadhouse bar with cas (and the harvelles) and and of course was meeting him on that stupid ride he took right away. How many times can a man mourn the loss of the love of his life? It had to get him sometimes.
