I feel like deaged Sam with all of his adult memories would be so fucking bratty. Always clinging to Dean’s side, going on and on and on about how he has the best daddy ever, constantly wriggling around on Dean’s broad, tightly jeaned lap, doing anything and everything he can think of to tease his big brother until he’s dragged to the nearest alley and saying daddy for a completely different reason.
Of course. All of this is dependent on the idea that Dean would ever let deaged Sam leave the bedroom when we all know that there’s less a 1% chance that he would.

Imagine. If you will. Sam gets deaged into after school special Sam. Dean is over the moon with joy. And lust. Sam would love it if he could be back in his own body again. But there’s something comforting about how hungrily Dean looks at him and how Dean won’t let him out of bed. It’s been about a month by now and Sam was still in his younger body. He doesn’t care though. Just so as long as Dean never pulls out. He’ll be fine. They both are.
My brain can't help but latch onto that "something comforting about how hungrily Dean looks at him" and make it angsty—like Sam thinks Dean only wants him like this, like his grownup self has become too damaged and tainted and Dean doesn't find him attractive like that anymore—and Dean's guilt when he realizes that's what Sam thinks. Dean trying to convince Sam he wants him no matter what, he just missed the Sammy that was small enough to carry with ease. But also maybe Dean's internal struggle over being a creeper because man does that face bring back memories, and they really have been going at it like rabbits instead of looking for a way to fix it.
I think the cruelest thing that could happen to Dean is if Sam was deaged to after school special age. And the only way to break the spell is for Dean to breed him. So now he’s torn between living out his fantasies and keeping his baby brother a baby for longer. What would he choose?

12/13 ish, maybe. 11 hard limit.
14 is typically my youngest age for sexual weecest (though there are some outliers) so that would be the age I think older/present!Dean would be comfortable with it. His body at 14 is on the cusp of a height growth spurt, he's got muscle development from being more involved in hunts, and he's more sexually developed (erections, orgasms) and his body can maybe accommodate intrusion better. It's the age Dean knew his little brother had an interior sexuality (thinking/dreaming about sex, jacking off, looking at porn mags, etc), so even though he'd look a lot younger relatively to older/present!dean, Dean back then hadn't thought of Sam as a child anymore.
Imagine deaged!Sam, with his memories, with a deaging curse that progresses younger at an increasing rate, finding out Dean's limit in real time. Sam disappointed and annoyed because he's still a grownup inside, but also relieved Dean does have a limit. Surprisingly relieved, because he didn't really think Dean looked at kids like that, but the younger Sam got the more his unnamed anxiety had grown.
I don’t know why but I feel like sex could have broken the Zachariah spell. Dean Smith pushes into Sam Wesson and boom. Dean Winchester is back inside his baby brother again. Sam Winchester is underneath his big brother again. Just like they should be. And Zachariah kicks himself because he thought of everything EXCEPT for incest proofing the illusion.

I also like the idea of Sam having the boom realization, because he's already had creeping deja vu, but it finally creeps in for Dean because of this and still takes most of the way through before he remembers. 👀 He's (unfortunately) always slower on recognizing Sam in these types of identity situations
