The Exam Begins. This is one of my favorites... it's a running monologue of what goes on inside the kinky gynecologist's head while he comes in to give you your exam. A Surgical Interlude (Part 2)

A few minutes later I pop my head in. You notice I'm wearing my surgical scrubs, I've got my cap on and mask, but since I haven't scrubbed in, I haven't gloved up yet.

"Bring her in please, let's get her positioned so we can start promptly at 9"

"Yes, doctor"

The nurses that were checking various things, your IV, your chart, began to busy themselves. You find your bed becoming mobile, items being attached to it for the short voyage down the hall.

"So she's not going under then?"

"No, not with the spinal."

"Poor thing, who wants to listen to doctors talking about how they're cutting into your body! Especially your lady bits"

The other, older nurse just realized they were talking right over you and tried to cover for the slip.

"Don't you worry poppet, you'll get to relax and it'll all go perfectly fine. We'll be monitoring and taking good care of you, nothing for you to worry about at all!"

You kind of nod in agreement, but she'd already focused back on her own job. Soon enough they're rolling you down the hall. Going head first -- backwards, really -- you're disoriented, and can only watch the lights roll by overhead until you reach a pair of large, heavy doors and one of the nurses pushes a button and they open automatically. They roll you in and you feel a chill -- it must be 15 degrees colder in here!

The large room with white tile everywhere is clearly your destination where they'll take care of your cyst and do that rectal exam he was talking about. You shudder briefly at the memory of the enemas. What on earth were they going to be doing back there anyway?

"We're going to move you over to the operating table now, just be still and we'll take care of all of it, you don't need to do a thing."

Before you know it, the staff lifts you up by the sheet that's underneath you and places you on the cold, black table with all the attachments. There's a couple blocks behind your shoulders you notice. The nurses each grab and arm and place each on the arm extension to either side that run nearly parallel to the table along your sides. One of them is messing with your IV, to hook up other medications should they be needed, and the other is strapping down your arm.

"Do you ... have to, umm, strap me down? I won't actully feel anything, right?"

"Of course not, love, it's just to make sure there's no extraneous movement during the procedure. Entirely routine, I promise. If you feel uncomfortable with it let us know and we'll see what we can do, okay" You can tell she's offering, but they're probably not going to unstrap your arm for just about anything short of a panic attack. Maybe not even that.

At the foot of the table another assistant -- male, you notice -- is lifting your ankles up and placing them into slings a couple feet above and on either side of the table. You can't feel a thing down there, but your brain imagines a cold breeze caressing your private bits as they become fully exposed for the first time. Another assistant is placing drape after drape over your legs and abdomen, until pretty soon the only thing you imagine is visible is your genitals and butthole. Nothing for anyone to look at, at all, except your most private and embarrassing parts of your body. Great.

"All right everyone, ready to begin?" I look around at the rest of the staff and confirm they're prepared. One of the nurses lifts one last drape up and clips it high, hanging vertically right over your abdomen. You can't see a thing below there now, and you can't feel a thing. All you have is the dialog that goes on around you and the remarkably unsettling distant pulling and pushing sensations that travel through your bones and body.

"Inserting speculum ... no, give me the larger one, I want as large a field as I can get, she's pretty small."

"Ultrasound please ... let's see her ovary up on the monitor. There she is ... and there's the cyst. No wonder she's hurting ...

It's as if you're not there. And, you might as well not be. You're not in pain, and they don't need your cooperation to do what they need to, but it's a team job. Just one nurse who's sitting next to you whispering calm platitudes in your ear...

Shhh, it'll be okay.

The doctor knows what he's doing. Don't worry about all that scary talk about scalpels and cutting and blood...

"#4 Scalpel...good cut. Sponge please. Looks like a bleeder, cautery there ... and there ... thank you."

Just relax, this will be over soon...

A few minutes later, it was.

"Okay, Jake, close up for me there. I'm going to start with the rectal examination."

You feel some tugging low on your abdomen where they needed to go in to work on your ovary. The internal route only helped them so much, it seemed. They're done in there, so just a few stitches outside, and ...

"Hand me the Mathieu please, and let's inject her sphincter with lidocaine and epinephrine." What, you wonder? Why? You look slightly alarmed...

Shhh, shh, you won't feel it, the doctor just needs the muscle to relax since he's going to be opening you up so wide ...

"There we are, and let's see ... adjust the light please ... there we go. Excellent. Okay, let's get the scope over here ... I want to zoom in there .. small hemorrhoid, nothing to worry about ..."

Well, you've got a hemorrhoid. That's news.

"Let's treat it anyway, cautery please ... done."

Well, you had a hemorrhoid.

"Another one there ... got it..."

Hemorrhoids.

"Okay, let's band that one, that's a big one. And please note on the patient's chart that she should increase her fiber consumption for the next few months until these clear up."

...... well that wasn't embarrassing. Okay, yes. Yes, it was.

"Removing the speculum..."

Guess that's all done?

"Let's use the sigmoidoscope now, make sure everything's clear up through the sigmoid colon."

Guess it's not...

You see me making motions like I'm inserting a tube ... you know where. And then you see your rectum up on a brightly lit monitor where both you and the doctor and nurses can all see it.

"Okay, rectum looks clear..."

"Entering the rectosigmoid junction"

The view on the screen has the appearance of going into a different cavern of a cave, and clearly a corner is about to be turned. You feel a little more pressure in your abdomen now.

"Turning towards the descending colon ... good view. Okay, inflating now ... "

You hear some squirts of air, like from a blood pressure cuff. and more pressure inside your belly.

"A bit more, please. Thank you. And working around the rectal folds ... looks clean so far. More inflation. There, end of the sigmoid. Nothing going on that I can see, let's take some pictures and get out."

There, the doctor's nearly finished...

You see everyone relax just a little bit after the doctor's announcement and small signs of cleaning and straightening up are occuring. In a moment I walk up next to you.

"You did great, I'm all finished. Your ovary is fine, and the cyst is gone. You'll have some pain for a month or so as it heals, but not like you've had. I did find a few hemorrhoids inside your bottom, however, so I'm going to recommend a high-fiber diet for a while and have you make some follow-up appointments monthly for a little while so we can keep an eye on things in there, okay? You'll hear all this again later when the medications have worn off, but I wanted you to know."

"Th-thank you, doctor..." I hear you say shyly, and you even blush a little bit. I smile and place my hand on your shoulder briefly.

"You were an excellent patient. Now, off to post-op with you and I'll talk to you later."

im waiting and waiting in the pre-OP area. the nurses say it will be my turn soon and that nasty cyst will be taken out. i can’t feel anything from my tummy down! my nipples feel tingly and weird too, but the nurse said that was normal.

Hi Doctor, I know this isn’t your usual topic but could you help a girl with a potentially broken toe? I hurt it last night and I don’t know if it is sprained or broken but it is ouchy…

Goodness, that sound painful! I’m so sorry that’s happened to you…

Just let the doctor take a look… I know it’s ouchie but I need to see and touch it just a little bit. Don’t be scared, I’ll be gentle as I can be.

Now, if it is just jammed, it’s like a bruise at the tip, and should clear up in a couple hours or a day or two at most. Probably not much swelling so it should look pretty normal.

If it’s sprained then you’ve stretched some ligaments attached to your toe. You can see if you can move it at all or put your weight on it. If you can, that’s a good sign it’s just a sprain. Also if there’s some swelling but not too much. A bad sprain could still swell up a lot but only bad ones. The pain will also be more generalized, so when I touch it … I know it hurts, sweetie, you’re being so brave! - it’ll be sore all around the toe.

Now if it’s broken, it will be probably a bit discolored and swollen, definitely. The pain will be a lot stronger and right where the fracture is, so if the pain is more diffuse it’s more likely a sprain.

I know it’s scary, but the doctor needs to look. Does it hurt when I touch there? How about there? Oh, baby girl, you’re such a good patient! I can’t be sure but I think it’s just a sprain, so let’s get some X-rays taken and make sure.

Thank you for coming to ask and to trust me to examine your sore toe. That’s very scary and by being brave you were a wonderful role model for all the other girls and women here!

And I actually feel bad when some of the darker or more vivid, perhaps scarier writings come up and maybe put off those of you who aren't here for that. But, as I only have one blog (and plan to keep it that way), they can't help but get mixed together a bit.

Please know this -- my first patients were scared, sensitive littles, and you will always be my firsts and most special. Of coure you are welcome and I would absolutely love to examine you. For you I would be especially gentle and kind! Although, there would definitely be plenty of embarrassing parts, I think...

Since I knew I'd need to take a deep look inside your bottom I arranged for you to get a few enemas before your exam. Hoo boy, did your face get red when you heard about that. And again when I popped in to check on your while you were still expelling the second one. For a moment I thought you'd actually yell at me to get out, but then didn't want to draw any more attention to yourself than the waterfall coming from your bottom.

You did look nervous in your pink gown with the kitties on it when eventually you were ready and I came in for your exam. There was a blank on your exam chart, so I asked you if the nurse took your temperature yet. You shook your head.

"We'll take care of that now then, sweetie," I announce as pull on a pair of gloves. I fetch the probe from the wall thermometer and pop it into a new probe cover. "Roll onto your tummy for me," I say, and you realize where the thermometer is going

"But -- no -- I -- we always take it in my mouth at home," you say desperately.

"And you're not at home now. Here we take it in your bottom, rectally, like a good doctor should. Now, are you going to roll over for me or am I going to have to put you over my knee?" You definitely didn't like the sound of that and promptly rolled over.

"See? Listening to your doctor isn't hard." You refuse to look back so you don't see me lubricate my finger. "Need to check for any blockages first, relax your bottom for me sweetie," I say as I untie and open your gown and then spread your bottom cheeks apart. You squinch up your face and make a noise, then you feel my finger at your little rosebud. "Little pressure, deep breaths and bear down for me, just like on the toilet," I say gently. Your eyes open a bit at that, but I feel you relax and your sphincter relax, so I push my finger in, taking advantage of the opening. You mewl in complaint, but I'm only in there for a moment, and before you know it my finger has been replaced by the cold, hard, but still slippery thermometer probe. Your face is buried in your hands now, but after 15 seconds the thermometer beeps and out it comes.

"98.2" right in the healthy range for a healthy girl," I say as I dispose of the cover.

"Sit up for me now, sweetheart, and we'll get going on your exam."

i reallyyy like the idea of being given just enough sedation to not fall asleep but feeling no pain and not being able to control my body. feeling something but not being able to pinpoint it through the haze of the drugs. and hearing the doctors and nurses talk!

and always gyno procedures, and catheters too. and for me it doesn’t end with the surgery, because there is the aftercare too. waking up from anesthesia, all emotional and scared. having nurses and doctors coo at me and checking the surgical site etc. the surgery might be over, but that doesn’t mean the nice doctors and nurses are done spreading my legs and doing what they want to my princess parts

Well, this ran a little bit long. Not sure if it's any good, not quite what you were looking for, I don't think. Sorry, my mind finds a good introduction to a story and it takes on a life of its own. You were so squirmy in the ER when they called me in for your gynecological consultation. So tender you hardly let me examine you vaginally so ... I had to go in your backdoor. You weren't happy about it, but it did prevent all those other excuses you made. True or not, sometimes needs must, and when I needs to examine a woman with severe lower abdominal pains, I must examine her. Oh, the faces you made when my finger was in your bottom though, those were precious. And when I said you had a ruptured cyst and we'd need to operate immediately, your face calmed down. Almost like you were happy with that answer.

Not many girls like heading into the OR to have their girl parts worked on, but you did, I could tell. I suggested we just do a spinal for you so you could appreciate the full event. Your eyes just about popped out of your head at that, but then I mentioned we'd sedate you for it as well. Just keep you calm, but aware. You relaxed, and your ... fear? excitement? arousal? ... whatever it was, it seemed to subside a bit. Though then your eyes started darting around the room like you were trying to take it all in for your future memory scrapbook.

I popped in to see you during pre-op. They were putting your catheter in at the time, so I ... supervised.

"These are just antiseptic wipes, they'll feel a bit cold..."

You definitely blushed when you saw me step into your curtained off area. I especially enjoyed watching your eyes when the nurse dipped the catheter into the jelly and held it up in front of you.

"N-no numbing stuff, or anything?"

"No, we don't usually do that in the OR. Of course, we usually don't do that before the patient is under anesthetic, but your doctor insisted we have you catheterized before being brought in. Is that okay? I can get some if you'd like?"

"Oh, ah ... no, I'm ... sure it's fine...?" So cute how you pretend, feign fear ... oh, it might even be real, but I can see the excitement and anticipation underneath it. I've been doing this a long time and I've always enjoyed working on girls like you. I nodded to you as the nurse began to insert the catheter. Your eyes popped open a little wider as you realized ... I knew.

"Hold still sweetie, a little burning now..." You winced as the catheter first entered, then moaned a little. An odd combination. She pushed it deeper ... more whimpering. And then, "Just a little pinch here as we get to the bladder," and ... you squeak. Yes. You literally squeaked. Just like a little mouse.

"Such a brave girl, isn't she Norma?" I say to the nurse who's taping up your catheter and straightening up.

"She certainly is! I've seen plenty of women cringe and fight me on those before, and they even had the lidocaine laced jelly! She wasn't as happy about the enema though, were you love?" Your face goes deep red this time.

"B-but why an enema? I'm not even going to be out!, and you're ... you're not working back there!" You looked indignant.

"Remember I said there was some slight masses or possible endometriosis I felt during your exam? I just wanted to take the opportunity to look closer since we'd have you anesthetized anyway. We don't have to, I'd be happy to schedule an outpatient rectal exam instead ... probably a proctoscopy, actually." You shake your head quickly.

"Oh, no, I, ahh, forgot about that," you say quickly.

"Well, by the time she got to the third and last enema, she was ready to write to the PM and get me fired! Even though she said it wasn't uncomfortable, and ... did you say you kind of liked it, love? Just the indignity alone was enough to complain about!" Norma laughed.

"Well, yes, no one likes things in their bottom--" I pause for dramatic effect. You notice. "--but it's hardly worth making a national case over, I don't think. Besides, do you really believe the PM wants to hear about your bowel habits, pet?"

Suitably chastised, you slink back down in the bed, speechless.

"Didn't think so. Ah, here comes the anesthesiologist. We'll be starting the procedure immediately after he finishes, any questions for me?"

This time you do look a little scared, but you shake your head.

"Would you like me to stay with you?" I ask. You nod shyly.

The anesthesiologist comes in and lays out all his paraphernalia on a tray next to your bed. He checks you out, asks you a handful of questions to make sure there aren't any contraindications, and finally gets you sat up with your legs hanging off the side of the bed. The bed's high, and you're not tall, so they just dangle there a bit.

"Bend forward for me, darling, that's a good girl," he says as he opens your gown and starts disinfecting you for the procedure. You gasp slightly at the feeling of the cold disinfectant on your lower back. The nurse steps in to help you hold still, but I wave her off. "I've got her," I say, and sit down in front of you.

"This isn't a big deal, it'll be over in a moment and you won't feel a thing below your waist. Just look at my eyes if you need to, and lean into me. I've got you," I say and you look somewhat mollified.

"Big pinch, this is just the local," he says. You squeak again, but louder and more forcefully this time. "Ow ow ow ow ow!" you mutter to yourself, but you're being brave. I pull your head next to mine, and arrange my arms to keep your upper body rock steady. A moment passes as the anesthesiologist presses into the spaces between your vertebrae looking for the perfect spot for your spinal.

"Here we go, you may feel some slight sensation if the needle brushes against any nerves, but hold still for me," he says, as he begins pushing the 4 inch needle into your back. You whimper, but more out of a sense of fear than pain. I hold you tight, quietly reassuring you.

"Nearly done, you're doing great."

You cry out as you feel an electric shock run down your leg. I hold you tight.

"We're in. Just a bit of the medication now..." he says as he attaches a syringe to the needle and injects the numbing agent. Within seconds I feel your body slump slightly as you lose feeling. I hold you steady until he finishes up. Soon the needle is removed with a bandage over the tiny puncture in your back, and he's gathered everything and heads for the door.

"Thanks John, nice work," I say as he heads out. Gently I help you lay flat on the bed. You look at me strangely, like I wasn't the monster you thought I was or something?

"Okay, showtime. See you in a few?" I wink at you. You smile gently but nervously and nod, saying nothing back. Next stop, the OR ... where that pesky cyst will be fixed and ... apparently ... you'll be getting an extremely through examination of your rectum.

It’s quite surprising how many of my sweet patients that reach out to me are from across the pond in merry old England, or at least the UK…

What is it about the NHS that gives you all such complexes about coming to see the doctor, hm? Or is it just a land obsessed with fannies?

hi doctor! i’ve been in your inbox before and now im here again. you said surgical mask and i got wet…

my medkink leans much more towards procedures and exams in operation or sterile rooms. screens that cover up so that i cannot see what is going on between my legs. anesthesia through a mask is also one of my favourites… and preferably the doctors and nurses talk over my head.

do you prefer the office exams or do you see yourself as a surgeon now and then too?💗💗

Is that right, my sweet patient? Did the thought of me in a surgical mask about to begin working on you get your juices flowing a little bit?

Far be it from me to deny credit for such a glorious occurrence. If a little bit of talk about sterile procedure or operating rooms, well, I think I can come up with a few little tales to bring you through that you might like. I've been in a couple ORs myself, and I know what that feels like. It's not quite the thing that I like most, but I can certainly give you a little taste if you'd enjoy that.

First, though, I'll need a little bit more information, okay?

What sorts of surgeries do you find yourself most likely to need (e.g. what do you like to imagine)?

Do you prefer being put under, or would you rather get a nice regional block ... like a spinal, epidural, or pudendal perhaps ... where I could then do what I needed to while you're entirely awake and alert, feeling the pressures and movements but no pain?

Do you prefer the idea of surgeries between your legs? Gynecological ones? Or we could perhaps do a deep exploration of your bottom? Oh, there's just so many possibilities.

Are there any specific fantasies you'd like to read about coming true?

While I don't typically imagine myself a surgeon, the idea of doing a surgery on someone who is awake and alert is kind of exciting, isn't it? Actually cutting into them, doing things that would be horrid but they're totally without sensation where I'm working. But they can hear me ask for the #4 blade, or the retractor, or to hold the cavity open so I can cauterize or suture the tissue?

And I would of course enjoy seeing you catheterized for the procedure as well. Maybe not your thing, but ... it certainly is mine ...

Shhhhhh, shhhhhh, it's okay my sweet missing 'moji girl!

You're not Annie, and you won't have anything like that happen to you. Annie was a little bit of a special case, and one of my assistants got her to explain how she got a little bit worked up at the doctor's office and she got nervous, but enjoyed it, especially some of the scary stuff.

So I just arranged her treatment to help her enjoy it even more, in those ways that she wants and needs to.

But not you, my special girl. You're much too delicate and tender for anything like that. I'll get Crookshanks to cuddle you during your time with me. If I need to do anything scary to you I promise I'll take it super duper slow and tell you everything you want to know along the way. I won't let you see any of the scary stuff, and when the tiny little ouchy parts happen, I'll help you through it and tell you what an amazingly brave little girl you are.

And at the end of it all, you get forehead kisses, and a lollipop at the end, and ... if you were really good and brave for me ... maybe even a big hug?

So are we okay? I hope I didn't scare you off ... we have a new year coming, and so many things we can do together to have fun!

So I'm a little because of trauma regression, not because of a kink. Do these types of littles get different types of treatment or are they left alone until they're ready to play and actually want to have kink play?

As far as I'm concerned, it doesn't matter to me why you're little -- if you are, and you need me to be gentle with you, then you get what you need.

Of course, everyone's treatment progresses along at their own rate. No one gets pushed along faster than they're ready for. Of course, sometimes that may be faster than they want, or think they're ready for, but I try to be very careful to make sure no one's any more scared or nervous than they want to be.

So come by my office, little one, tell me what's troubling you, and tell me you're a little bit nervous. Tell me you want me to be gentle and go really slow, and not to push you until you're ready, and I promise I'll take the very best care of you.

Nothing weird, nothing uncomfortable until you're ready for it. That's my promise. And when you are, just let me know. You can tell me ... or, you could hint at it a little bit ...

"Doctor, ummm ... my bum has been a little sore recently, could you ... take a look for me?" ... and I'll be very gentle and give you a little more of that special attention that you need.
