Correct Answer: The chest x-ray is indicative of fungus balls, also known as aspergilloma. Of which, infection by Aspergillus fumigatus is the most common; typically without symptoms as it grows in the lung’s cavities without any interference from the body’s immune system. In a small percentage of cases, however, the infection can break through the wall of the cavities and cause bleeding leading to hemoptysis(coughing up blood).
Seen mostly in patients who are immunocompromised or have underlying lung co-infection such as Tuberculosis.

This particular "Challenge" relied on both the process of elimination and a bit of luck, as medicine itself isn't always clear cut.
Based on symptoms alone, you can rule out melanoma and SLE. Mycosis fungoides, unlike what the name suggests, is not a fungal infection but a type of Non-Hodgkin's Lymphoma. A French dermatoligists from the 1800s saw the mushroom-like skin lesions and termed it such.
Of the two options left, Shingles and Syphilis, a combination of patient's medical history and laboratory tests(a bit of luck for this challenge) will give us the right diagnosis.
Correct Answer: Syphilis, tertiary(stage IV)
