I like to think on how they managed to meet in Simon’s mind.

So here we go: The actual Simon does know about how David drove into him with his car, but the Simon who is wandering around in his mind doesn’t. So, because the real Simon knows about David, there also is a David wandering around his nightmares who meets the unaware Simon. Not sure if it’s the actual David. It also could be The Addiction (aka that boss fight David) because all of the monsters represent something relevant to Simon and Leatherhoff definitely should be one of them (I mean, he crippled Simon..)

Well yeah, that’s my headcanon.
