Community Services Block Grant (CSBG) Scholarship

Award: $500 to $3,000

Requirements:

Will be enrolled in Illinois college or vocational program full-time in Fall 2012

City of Chicago resident

Family income does not exceed the requirements (see the application for specific details)

Need: 200 word essay, letter of recommendation, official transcript, college acceptance letter, proof of family income, FASFA SAR, proof of Chicago residency, and family's social security cards.

Scholarship for Male Seniors

William J. Cook Scholarship Fund

Award: $1,000 to $5,000

Requirements: Male senior attending high school in Cook County; enrolled in full-time program at four-year college

Scholarship recipients are selected on the basis of financial need, academic record, demonstrated leadership and participation in school and community activities, honors, work experience, statement of goals and aspirations, unusual personal or family circumstances, and an outside appraisal. Financial data is reviewed to determine the amount of each award, ranging from $1,000 to $5,000. Financial need of a minimum of $1,000 as calculated by Scholarship Management Services must be demonstrated for the student to receive an award.

Dr. Alma S. Adams Scholarship

For students pursuing course of study in public health, communications, social work, education, liberal arts, or related fields. Need 3.0 GPA.

The awards will be granted on a competitive basis for the following: a) a record of commitment to community service on behalf of an underserved community, preferably related to tobacco prevention and/or control and b) the best use of the visual arts, media, creative writing or other creative endeavor to convey culturally appropriate health messages aimed at raising awareness of tobacco’s harmful impact.
