I just remembered a conversation I had a while ago with a friend who's studying medicine. It was about fatphobia, I was trying to explain how doctors fixated on weight loss instead of looking for a cause.

He asked how he should approach this, then, since it's not something that can be ignored in someone's health.

Here's what I remember:

'Fatness is not sudden. The patient lives with their weight day after day. But something changed in their health and that's why they came to see you. You need to focus on that change, because that's what causes them distress.'

So, next time you're at the doctor, and they want to make it about your weight, remember this. Focus on the thing that changed, that hurts worse than before. Say, yes, I'm aware of my weight, say these are long term changes, but you need them to focus on short term, now, because I'm suffering.

(One thing that works for me is to say 'Yes I've been making an effort, I lost five pounds' and then they shut up about it and can get to the real problem.)
