According to research from the Confederation of British Industry, 99% of all economic activity in the UK is driven by SMEs and by 2020, medium-sized businesses alone could contribute as much as £20bn - £50bn to the economy.

Yet as important as SMEs are, they are often seen as the enemy of “big business”.

£600m Arch bid to 'transform' west Wales NHS

The NHS in south-west Wales cannot continue to deliver services in the same way as it has done for 70 years, health bosses have warned.

They say "sticking plaster" solutions cannot tackle the "huge pressure" on services.

It comes as two health boards and Swansea University will soon submit ambitious £600m plans for transforming healthcare.

It includes an eye-catching £225m "wellness" village near Llanelli.

This lakeside complex close to the coast would combine health facilities, life science research, sport and leisure with a care home and assisted living.

Abertawe Bro Morgannwg (ABMU) and Hywel Dda health boards - responsible for one million people - are working together with Swansea University in what is being described as a "totally unique" partnership.

One of the most ambitious elements is a plan to build a £225m "wellness and life sciences village" - in association with Carmarthenshire Council - on former industrial land close to the Llanelli coast.

The concept, which is described as a "world first", would bring together a range of services which help people lead healthier lives and deliver care closer to people's homes.

It includes an eco-park, community health centre and rehabilitation centre but also a "wellness hotel" with spa - to try to tap into the emerging wellness tourism sector.

Why is CPR, an expert on fighting phone scams, now a Telecare provider?

The best answer is probably written by Roby Ellison (CTSI Policy Officer)  in  Priorities for the Health, Social Care and Sport Committee:

‘There is clear evidence that loneliness is linked with deterioration in health. Around a million (10%) older people are termed ‘chronically lonely’ at any given time in the UK. 

This seriously increases their risk of suffering mental and physical illness. Loneliness also puts individuals at greater risk of cognitive decline which is a key factor that contributes to an individual becoming a victim of a scam. 

Furthermore, victims of scams are often lonely and engage with a scammer more readily than other people because of their natural need for human interaction. In addition to the personal suffering caused, loneliness is heaping pressure on various local authority and health services, as it is often the tipping point for referrals to adult social care and cause for a significant number of GP visits.’

Government announces £300 million for landmark ageing society grand challenge

“Each centre will enable companies, including SMEs, to rapidly develop, test and implement products and systems in partnership with doctors and academics, improving patient care and gaining early evidence of real-world product value. “

Based on previous experience, the red tape will preclude the real innovative SMEs to get involved and allow the dinosaurs to peddle the usual out of date solutions.  

This is not cynicism, the SMEs are always asked to join in the fun once the often inappropriate pathways have been developed.  Swansea Bay investment anyone?

So how can we get an SME voice at the early stages so that the Government can deliver the necessary bang for the £300 million?

Unity programme

The European Community’s Unity vision is to strengthen the connection between the police and the diverse communities they serve to maximise the safety and security of all citizens. This is how they see it:

The end-user focus of Unity shall identify best practices in Community Policing (CP) through primary and secondary research to enhance cooperation between Law Enforcement Agencies (LEAs) and citizens through the development and live pilot demonstrations of technological tools in six EU member states that facilitate, strengthen and accelerate community and LEAs communications. 

These tools shall be amplified and supported by the design and delivery of CP training and awareness raising activities to LEAs, citizens and community partners, including online virtual communities. Unity will provide LEAs with a new CP model and shared framework of governance and enabling tools and technology to support closer cooperation for greater, more effective and efficient and more inclusive CP. 

The citizen-centred approach of Unity support the combined protection, safety, security and well-being of communities, but it will also support a more collective, shared ownership of large-scale, collective risk. 

Coordinated by pioneers and practitioners in CP, Unity seeks new ways of working in which the police will serve as a catalyst for change within communities, helping the latter to become an integral part of the solution, and thereby sharing the ownership and delivery of a sustainable CP model which simultaneously embraces the benefits of technology while meeting diverse community needs. 

This new and sustainable citizen-centred CP model will have community trust and confidence at its heart, with the ability for two-way flows of information and communication to allow for greater understanding of the problems and issues faced by communities. By working with citizens and community stakeholders to arrive at a full understanding of their concerns, targeted interventions and solutions can be agreed to keep local communities safe.

We are similarly working with local businesses and Universities to create the future of Telecare.  It is pleasing that the debate is moving towards developing strong innovative procurement channels rather than limiting itself to buying what is best on the shelves at any certain time.

Domestic abuse

Domestic abuse is linked to coercive control and we are very proud we can help the Police help victims of domestic abuse.

Domestic abuse is an “act or a pattern of acts of assault, threats, humiliation and intimidation or other abuse that is used to harm, punish, or frighten their victim” (women’s Aid https://www.womensaid.org.uk/information-support/what-is-domestic-abuse/coercive-control/).

We have recently worked with a Police force to enable victims of domestic abuse to retain a way of reaching out and contacting help, even if the level of coercion is such that victims cannot use the usual methods of communication.

This work is not part of what we do normally but we were hugely proud to, in our own little way, help the police stay in touch with those suffering from domestic abuse 24/7.

Obviously, we cannot say how we do it.  If this is an issue for you, challenge your telecare provider.

Social isolation, loneliness and technology

Local Authorities and care providers are recognising that their biggest challenges are Social Isolation and Loneliness. 

The stats (Holt-Lunstad) do not lie: you’ll have a better life outcome if you smoke 10 cigarettes a day or if you are obese or if you just stay on the sofa all day than if you are socially isolated (‘lacking social connections’).

Social isolation can, of course, be objectively mapped as it is quite literally about the people you engage with. Loneliness is quite different as it is how you see life.

Interesting social debate?  No.  Those who are socially isolated are more than 3 times likely to enter into local authority funded residential care. 

This social isolation is also increasing their likelihood of mortality by 26% as social networks help you recover when you get ill (Marmot, Michael).

As 100% of the population is likely to die at some point, is the increased likelihood in mortality by the socially isolated another social debate?  No.  In England, for example (according to Nuffield research), patients who are socially isolated are 60% more likely to visit the emergency department than those who keep links with the community.  This must be a significant portion of the £11 billion NHS yearly emergency bill.

The sad irony is that the growth of traditional telecare solutions forces individuals to remain at home to be safe, thus increasing their level of isolation.

So, what we do is, simply, come up with an answer that works for everyone…

The reason why our tech helps vulnerable people whether they are at home or out and about is because it is well-designed.  Greg also ensures all the products are tested before they are shipped.  So he does deserve some credit!

Why Golf is good to avoid groupthink

Andy giving the trophy to this year’s CPR sponsored Golf Tournament.  

Groupthink is about seeing what you want to see rather than seeing what is obviously in front of you.  Playing Golf helps you see properly!

I surprisingly found myself surrounded by a congress of trading standards officers recently.  Whilst waiting for prizes to be awarded after our CPR golf tournament, I was interested to hear how different people who essentially have the same job throughout England and Wales actually suffer from the same tunnel vision.

They shared the same problems and challenges.  They also universally shared the same solutions.  As a group, it is fair to say they approach problem-solving in the same way.

So, reassured that they were unlikely to throw me out of the room now that they ate a very nice curry, I challenged their approaches.

We discussed 20th century thinking with 21st-century technology.  We discussed the gap between what 21st-century care ought to look like and what it is.

By the end of the conversation, different people had agreed to work in different ways.  I am looking forward to next year’s golf competition as the group think is now really broken and we are going to learn a lot.

Maybe I’ll buy myself a stick and even join them on the pitch...

The state of Telecare today

Boosting community resilience is about empowering communities by leveraging the power of the individual.  In many cases, such as after a fall, the immediate community is more likely to provide instant help than a call to 999...

Well, the Open University recently ran a conference on health and wellbeing and was keen to share the idea that technology is just not working.

Vulnerable people are not happy to wear stigmatising alarms.  Our work with a French telecare company highlighted that these alarms are rarely worn anyway. Traditional alarms increase isolation as they have limited reach outdoors.

The ‘alarmisation’ (new word: stick with me) of the vulnerable has hugely increased the pressure on the emergency services.  In Northumbria, the Police identified that 85% of telecare calls were ‘false positives’ (false alarms).

So CPR Global Tech is working with the University of Swansea, the Metropolitan Police, the FCC and FTC in the USA, a number of Trading Standards agencies and Academic Health Science Networks (AHSNs) to deploy sustainable community-based telecare.  We are calling our drive: Boosting community resilience.

This is a bit of a mouthful, so we call this Care 360.

We have a way to reduce incapacitation after falls, but our work is increasingly about breaking down isolation.  We do not only want people to live longer, we want them to be thriving for longer.  This will keep them happy and healthy in their own homes.
