I admitted someone while on nights who came to ED with shortness of breath and orthopnea for the last month, BNP in the 1000s, pleural effusions...admitted to evaluate for new diagnosis of CHF. There was hilar/mediastinal lymphadenopathy on CT, patient was way too young with not many risk factors for heart failure....called my shot and suggested sarcoidosis in my note.

The echo came back with an EF of 10% (!!!!) and patient got transferred to a higher care center for further evaluation and they think it could be due to SARCOIDOSIS. All that learning for Boards back in med school actually helped me 😅

My pregnancy hormone finally went back to negative this week. I’m glad that the weekly lab draws can stop and it’s finally “over.” But it’s also unbelievably painful.

I’m so Not Okay that it’s a wonder the people around me have no idea.

Have you been working with any people who have had covid?

They try to keep the residents on the non-COVID team but I’ve definitely seen people who ended up testing positive. One example was I was doing labor and delivery and rounded on a mom who had first tested negative and then tested positive after I saw her.

This is one of the many reasons you don’t make seemingly innocent comments like this to women. Because we don’t say, “I was pregnant and lost it, had to get methotrexate for pregnancy of unknown location, and have been bleeding for almost 3 weeks now.” Even though that’s the truth.

So shut up. Please. Just. Don’t do this.
