Question:  Does exclusive breastfeeding change early early brain development?

Answer:  Yes.  Brain imaging evidence supports early, healthy brain development in exclusively breastfed children.

Study design:  Using a quiet magnetic resonance imaging (MRI) scans, the researchers measured the amount of white matter (nerve fiber development) in 113 healthy children from ages 10 months to 4 years.  One group was exclusively breastfed for at least 3 months; second group was fed formula; third group was fed mixture of formula and breast milk. 

Result:  MRI study showed that breastfeeding increased growth in language, emotional, thinking, and motor skill areas.

Low Immunization Rates in NICU Babies

Call to action for parents with babies in NICU:  Be sure to discuss your baby's immunization before discharge. Journal of Perinatology published this study last year conducted by A M Navar-Boggan.  Dr. A.M. Navar-Boggan's group reported that of 668 discharged infants' data studied retrospectively from six NICUs in Northern California, with average age of the babies 60 days, 27% of infants had received no vaccines. Dr. Navar-Boggan and her colleagues urged for higher immunization rates for babies graduating from NICUs.

Obesity and Pregnancy Outcome

Number of patients studied:  25,562 women from 15 centers in nine countries

Question asked:  Does Obesity alone or Obesity + Higher Maternal Glucose (blood sugar) level influence the baby's health?

Conclusions:  Being obese during pregnancy negatively affects baby's health.  Being obese and being gestational diabetic (GDM) causes higher risk to the baby.  Babies born to obese and obese/diabetic mothers suffer from higher birth weight (big baby problem), greater body fat composition (over 90% of baby's weight is due to fat), C-section (baby is too big to pass through the birth canal), abnormal protein level closely associated with insulin (babies can suffer from low blood sugar).  Mothers are also at higher risk for preeclampsia (high blood pressure during pregnancy) and birth-related complications.

Study of Nicotine Use during Pregnancy Inconclusive

A study published March, 2012 in the New England Journal of Medicine looked at women using Nicotine patch during pregnancy.  The study recruited 1,050 pregnant patients.  521 were randomly assigned to nicotine patch therapy, and 529 to the placebo groups.  The study's objective was to find out if nicotine patch use during pregnancy was effective and safe.  Unfortunately, due to low compliance (most of them did not stick to the program beyond one month), the study was inconclusive.

I would like to share with you a new published study supporting breastfeeding.  This is a quote from MedScape medical newsletter update published this week.  A study from American Journal of Respiratory and Critical Care Medicine published 2/3/2012 noted that breastfed babies had better lung function when they were tested at age 12.  This is especially true for babies whose mothers had asthma.  Study followed about 1,500 children in U.K. born between 1993 and 1997.  When they were 12 years old, their lung function were measured by:  FEF50 (forced mid-expiratory flow rate which measures the speed of air breathed and tests the airway activity), FVC (forced vital capacity which measures how much air is breathed out), and FEV1 (forced expiratory volume at 1 second which measures the amount of air breathed out at 1 second).  Breastfed children did better in all areas of lung function measurement.

So, all of you expectant mothers thinking about breastfeeding, don't give up.  Persist.  It's worth it.

Studies show that asthma, diabetes, obesity are more common problems for babies who are not breastfed.  That's because of protective immunoglobulins (proteins that help fight germs and teach the young immune system to be strong) supplied by the mother's milk protect the baby.  It's curious how your vegan problems helped you.  May be you had allergies to animal proteins.  Do you also think you had trouble digesting animal fats?  It sounds like vegan diet really helped you feel better.  Good for you for solving your own problems.

That's okay.  Not giving up breastfeeding will help your baby and you.  It's not easy.  Follow your instincts.  Can you consult a lactation consultant, an IBCLC (Internaitonally Board Certified Lactation Consultant)?

Breastfeeding women should ask more questions

Breastfeeding women should ask more questions about how their nursing pads work.  For instance, they get sold on cotton, wool, or bamboo.  Many women think natural means good, also means healthy, also means whatever they equate as what's good for them.

However, they forget to ask:  Is it good for my skin?

Why aren't they asking questions like:  How long does it stay wet on my skin?  Does the water proof layer breathe?  Do bunch of germs stick to the pad, which also means my baby's getting my germs, too?  Does it pill up and cause abrasion (chafing) on my delicate skin?  Does it have seams or pleats to dig into my skin?   At the least, they should ask, "What will my skin feel like wearing this type of nursing pad?"

Coming from wound care medicine, I recommend a breastfeeding woman asks them before purchasing nursing pads.  All too often, they blindly pick up the cheapest or what sounds familiar.  Often their skin may not feel good using disposable nursing pads.  More women should ask if what they purchase is good for their skin.

No-calorie beverages aids in weight loss

A preliminary finding by Dr. Tate's group concludes that replacing sweet drinks (has calories, such as fruit juices, sodas, any syrupy drinks) with water or no-calorie drinks helped a group of mostly women to achieve weight loss at six months.

Those in the water group also had lower systolic blood pressure and fasting blood sugar compared to the control group drinking sweet drinks.

This is only a preliminary study and needs to be peer-reviewed.  However, given that about 300 calories per day can come from sweet drinks, eliminating this source will gradually reduce weight by reducing refined carbohydrate intake.

This simple practice coupled with some exercise are a lot easier to follow than the perpetual dieting.

First shipment was sent to help the hospital promote breastfeeding.

A recent Center for Disease Control report indicated that many hospitals fail to adhere to breast milk only policy for the newborns.  If a hospital wants to go Baby Friendly, it has to practice exclusive breastfeeding unless compelling circumstances prevail.

Breastfeeding Pearl to share

When I attended Miss Kittie Franz's talk on Supporting Breastfeeding through Communication offered by San Diego County Breastfeeding Coalition, l learned a pearl from a Question and Answer session.

In response to Miss Franz's comment that an area called areola makes special scent-producing chemical that the baby can sniff and locate mother's nipple, a lactation consultant in the audience said the following:

The breastfeeding mother's breast produces a unique fragrance that her baby can locate as milk source.  This fragrance is missing in formula, which is made either from cow's milk or soy milk.  This chemical sends the signal to the baby that his food is near.

Miss Franz also commented that "nipple creams" should be applied only on the nipple.  Creams and ointments can cover up the areola and hide the fragrance-producing glands.

San Diego County Breastfeeding Coalition sponsors Kittie Franz, RN, CPNP-PC

Part 1

On 8/6/11, San Diego County Breastfeeding Coaliton, www.breastfeeding.org, invited Kittie Franz, the renowned pediatric nurse practitioner, a faculty in the USC Keck School of Medicine, to present "Mother Baby Communication:  Facilitating Early Breastfeeding".

Her presentation covered the importance of Skin-to-Skin in the Hospital to promote breastfeeding practices.

Her talk was empowering, because she covered a concept called skin-to-skin.  When newborns are skin-to-skin with their mothers immediately after birth, healthy babies use in-born reflexes to find their source of nourishment, mom's nipple.  They use their arms and legs to step and crawl on their mom's chest, right their head and gradually move over to their milk source.  Some babies can do this with their eyes closed.  This Baby-Led Latching is the accepted norm for many professionals and veteran moms who have breastfed their babies.  However, for new moms, they should feel courage to trust their baby's instinct to find their milk source.

Miss Franz thinks focusing on "method of attachment" is counter-productive.  One person's method may be different from someone else's and confuse the new mother.  Instead she advocates Baby-Led Latching.

She also emphasized various positions a baby may choose to nurse.  Newborns have esophageal reflux, regurgitating food and stomach juice when they are either over-fed or in flat position.  Some newborns choose to nurse in vertical position.  Miss Franz noted that large stuffed pillows that force a baby into a flat body position is not good for newborn, because it makes the reflux worse.  Instead, a mom should recline at 45 degrees (a lean-back position), have the baby lie on her chest skin-to-skin (no padded nursing bras), and allow the baby to find his natural position.
