As a person with psychosis, I think the word "insane" used casually is fine, actually. Like. We need words to describe when someone is acting as if they are not in their right mind. Because people do that all the time. But the line comes at using words for actual symptoms/disorders to mean "bad person/action." Saying someone is "psychotic" when you mean "a bad person" or accusing everyone you don't like of being a sociopath/narcissist crosses the line (and no I don't care if you were abused by a mentally ill person. So was I by multiple people that does not give us the right to use those words that way).
