Do you have any tips on managing (and or avoiding if possible) diabetes as a gainer? Not sure where to look or where to start.
One of my first jobs after I got my masters in medical anthropology was working on a team designing a culturally inclusive type 2 diabetes prevention program. I learned a lot from the nurses and doctors I worked alongside. I’ve also learned a lot from having diabetes for the last 10 years while also gaining about 300 pounds on purpose.
Prevention
A lot of this is also good advice for managing your blood sugar if you already have a diabetes diagnosis.
1. It’s not always possible if you are genetically predisposed. But you can definitely delay the onset.
2. Physical activity lowers blood sugar and decreases insulin resistance. It doesn’t have to be arduous, even just going for a walk a few times a week helps. Try to find something you enjoy. I like to swim laps at the gym.
3. Eating balanced meals and choosing complex carbs over simple ones. This means whole grains and making sure you have protein, fat and carbs in roughly equal amounts in each meal. It’s never a bad idea to have more veggies.
4. Don’t freak out about a pre-diabetic diagnosis. This is a controversial diagnosis and is not accepted by the WHO. About 50% of those who receive this diagnosis go on to develop diabetes. It’s about as predictive as a coin flip. The positive is that it can serve as an impetus to try some of these strategies to prevent or delay actual diabetes.
Management
1. Don’t freak out if you are diagnosed. This is an easy disease to manage.
2. Don’t neglect managing it. Uncontrolled diabetes has significant consequences like blindness, amputation etc.
3. Losing massive amounts of weight to “cure” diabetes doesn’t work. You may see a temporary return to normal blood sugars but you will regain the weight back just like 90% of dieters do. And when you do regain the weight your blood sugar control will likely be worse than before.
4. Ask your doctor for a continuous glucose monitor CGM. They are probably free through your insurance. They are a wearable device that transmits glucose numbers to your phone. They keep you from having to constantly prick your fingers.
5. You have many non injection options for medication. Lots of different pills.
6. Insulin works great. Many doctors will want to delay starting you on it or push weight loss meds. This is because most people gain weight on insulin. Another medication that works really well and also causes significant weight gain is Actos. This medication actually helps move visceral fat out of your liver and turns it into beneficial subcutaneous fat. When combined these can contribute to rapid weight gain and improvement in blood sugar levels.
7. You are the person who decides what medications and treatments you go on. Not your doctor. There may be limits set by your insurance but if you don’t want to take a particular medication and want to instead try something else you have every right to tell your doctor this. You can also tell them you are not interested in losing weight. You can also decline to be weighed at the doctor. They are providing you a service, they are not your boss even though they have power in the interaction. This is called patient’s right of refusal and you should exercise it.
8. Be patient and kind to yourself. Anxiety around a new diagnosis is normal. It will take you time to learn how to manage your diabetes. Managing other issues such as your mental health helps make controlling your diabetes much easier.
Why I've done this (Part 2)
The early 20s As I started a working life, my weight eventually settled at 160lbs. But all of it changed around winter 2017. I had started to face my demons, one treatment of which was "Mirtazapine", that initially caused a fair bit of weight gain, especially at first. At the time I was working at McDonalds.. I have a stronger desire to eat when happy, I was taking a drug that made me hungrier, hornier and very happy... so in almost the blink of an eye I gained my first 30lbs. However when I looked in the mirror, it wasn't horror that flew across my face but instead there was a smirk. It was at this moment where it finally begun to click. So from there, I sent it. I started purposefully getting into gaining and in the first year I went from 160 to 260. Every lb I gained increase that smile, I was loving the changes and the confidence was apparent. The Mid 20s Eventually my struggle stuck at 260 turned into desperation and my intake started getting a big dangerous. a 2L bottle of full sugar pepsi ever day, countless sweets and chocolates.. I was likely having 2000% of my daily sugar a day, but it was working. My weight crept ever higher and the bigger body was always so very appreciated, and at 299lbs.. devastation. ... Many people don't know that the official condition is actually called Diabetes Mellitus, Type 2. I had all the signs my dad had. Dehydration, frequent peeing at night etc. I had a choice to make. I could lose the weight, go back down and just be a lil guy again at the hopes of reversing it, or I knowingly dive into the deep end, essentially medical insanity as I willingly try and dance a fine line between life and death (Yes this is quite the exaggeration, it's just how I felt at the time). Initially I chose to lose. Over the next few months I dropped down to 260 again and man was I miserable. It was like all the joy from my life was gone and I was starting to look at my body with shame. It wasn't great. I decided that enough was enough. I fought tooth and nail with the NHS and eventually was put on new medication which would help steer me in the direction I wanted. Over the next 2 years I slowly pick myself up and even go beyond, hitting an eventual high of 330lbs despite a few unfortunate events inbetween. Then I went for it, I fought hard to get put on insulin. Having only ever felt a stronger and stronger resolve to keep going and wanting to properly control my blood sugar, this was a leap to truly aim for my happiness without limits or blockers. Since then I've gained incredibly rapidly. Ultimately, I've gone from 340 to 520 in about 3 years (again with some losses in between), a gain of 180lbs. Every pound since that has only continued to feel more right. Even as my body slows down and my mobility begins to be more of an actual disability than an inconvenience, I've still never stopped adoring the increase in size. I genuinely feel that if money were no object, and care was bountiful, that I would gain to try and be the biggest human to ever live, maybe even aiming to be the first to officially pass 1500lbs. Could I stop myself? This is a good question. In the short term, easily. I'm not really a massive feederism person and I don't get much pleasure out of food unless im already in a great mood (which I VERY rarely am), so cutting down and moving more would be easy at first.. However the concern would be if the weight ends up going down, could I resist that desperate urge to go back up? Being over 500lbs before even turning 30, I could happily find a way to justify sitting at 500lbs and enjoying what I can with my mobility. But the other end of the scale, it's very much a call of "you've come so far, why not see just how far you can go?"... I don't know which side will win until I have to pick one. At the moment I've been blessed enough that I've consistently been able to go down the "keep going" path without even needing to think about stopping.. but this is likely to change. Between mobility challenges and rising costs I simply can't keep it up forever on my own.
What I can say, is that I've never felt so at one with myself. The gap between who I see deep inside and who I am at present has closed with each lb. I love my larger body, I just adore it. The weight, the heft, the softness and the size and the shape. My incredibly lucky proportions.. all of it.
What is it like to go to the doctor and get a physical at this size? Do they ever lift up your belly or examine it at all? Does that turn you on?
No, they don’t touch my belly. I’m expecting that someone will want to measure it at some point, but it hasn’t happened yet.
My GP is a big lad - he’s around 300 lbs, and he has a chubby nurse who’s around 250. The receptionist is a massive woman - easily 450-500 lbs. They’re all very friendly, and when I arrive the receptionist tells me that they’ve “got the thigh cuff” ready for me.
My endocrinologist is thin and tells me I need to lose weight, but his primary concern is what my numbers show, and they are all fine, albeit with huge amounts of insulin and other treats.
THE MALE FUPA GUIDE
What they are, how to get one, what it MIGHT do to you and those you love
Note: this does not constitute medical advice. I am a lawyer with an understanding of anatomy and forensic science. I have no medical training or formal scientific training.
1. The name
The term I use is “FUPA”. In some medical literature it is a “suprapubic fatpad”. I have seen it referred to by other names. I particularly like “junk-chunk”. I think that most people who have ever thought about FUPAs for any length of time have concluded that we have no satisfactory, everyday word for them.
2. What it is
It is a lobe of fat that appears beneath the abdominal crease. It may extend as far down as bottom of the scrotum. Generally the area that we are considering, though, is the fatty covering of the pubic bone.
3. How to get one
To get one, put on fat. The kind of fat you’ll need to gain is subcutaneous fat, and in the correct location. This is influenced in large part by genetics, but all is not lost if genetics do not favour you.
There is some evidence that you can shift from visceral fat deposition to subcutaneous fat deposition. A diet high in fat, as opposed to carbohydrate, induces less of an insulin response, and less visceral fat storage. The likelihood of that fat being deposited subcutaneously increases, as correspondingly, the likelihood of the it being deposited into the suprapubic lobe increases.
Additionally, even if you happen to gain a lot of fat viscerally, once your body reaches the point of having packed out your abdominal cavity with visceral lard, it seems to shift to increasing subcutaneous fat deposition.
So, even if you start off with the “wrong shape” for a FUPA, it doesn’t mean it won’t happen.
4. FUPA growth
Apart from people who are blessed with naturally big FUPAs, most of us will see slow growth at first, and then sudden growth once we reach a tipping point. There are a few reasons for this.
First, the suprapubic fat lobe is influenced by surrounding fat. As the belly grows, it pushes down and out on the FUPA, increasing its projection. Additionally, as the thighs fatten, they swell out and (somewhat) upwards, also increasing FUPA projection.
The process of gaining weight in the lower abdomen also affects skin tension. The fatter you get, the more your flesh tends to hang.
These effects are small at first, but combined with a slowly growing suprapubic fat lobe, can produce a runaway train effect on FUPA growth.
5. Measuring the FUPA
Put a measuring tape above the base of your penis against the pubic bone. Measure out how far the FUPA projects.
I’ll use the following classifications:
< 1 inch: minimal projection, does not contact penis basis
1-2 inches: Moderate projection; may partially contact penis base during movement
2-3 inches: Likely partial coverage
3+ inches: Significant fold; can fully obscure the base
6. FUPA Mechanics
This is the fun part. Note that it is not always possible to isolate the effect of the FUPA from surrounding fat in the belly and thighs.
FUPAs have a variety of possible effects. Not everyone will experience all of them. Some of the effects seem contradictory at first.
First, the FUPA will almost inevitably increase temperature in the area. A warmer environment may make your nerve endings more sensitive. The effect progresses as the FUPA grows. Even a moderate projection will produce this effect.
Secondly, as FUPA projection increases, it has the effect of pulling penile flesh forwards. For the uncircumcised man, this can result in the foreskin feeling especially fleshy, and actually impede erection, as the penis may feel tight and trapped. This may increase sensitivity or stimulation in some people. In others it reduces it. Even a moderate projection will have an effect on penile flesh.
Thirdly, the presence of a large FUPA generally contributes to reduced bloodflow to the penis, producing softer and shorter erections. Even a small FUPA can have a significant effect here; conversely, there are some men getting around with a two-pound fleshlight enveloping their dicks, and seem to have no problem with engorgement.
Fourth, the FUPA may, at around the moderate stage, begin to have an effect on the dorsal nerve. The dorsal nerve tells your brain what your penis is feeling. What exact effect it has varies, depending on the rest of your surrounding fat and your posture, but light contact, such as happens during movement, often produces sensations of tingling or electric flashes; some postures, such as sitting, leaning forwards, can also produce a burning sensation or numbness. What the effect is varies from person to person, but tingling sensations, and sometimes electric flashes, are often experienced as intensely pleasurable.
Paradoxically, involvement of the FUPA on the dorsal nerve generally results in a *reduction* in actual sensitivity to stimulus to the penis. Why would it be pleasurable in that case? I liken it, in my lay understanding, to the feeling we all have had of our arm just starting to fall to sleep. When that happens, our hand might be buzzing and tingling - and felt in a sex organ, that is intensely pleasurable - but if we actually touch our hand, it will be slightly numb.
Fifth is actual contact by the FUPA on the penis. This tends to occur only when there is a significant FUPA of greater than 3 inches. Most men seem to experience a reduction in sensitivity to lighter contact, and less-favourable angles for penetration. However, some men find an increase in sensitivity to firmer touches, and a sensation of enveloping heat and fulness during orgasm.
Putting all of these things together, there are many variables. Some will operate to reduce sexual pleasure. Some will operate to increase it. Whether the net effect is that it increases your pleasure is something that will be unique to you, and depend on how you prefer to fuck.
7. My own experience
My own body began to fatten with a big visceral deposit. The rest of my frame was somewhat muscular. I didn’t really have much of a FUPA until I hit 300 lbs.
Starting at around 350 lbs, I had a moderate FUPA. It grew rapidly to cover my shaft, up to the head of my penis, by around 400 lbs. This coincided with starting insulin therapy for T2 diabetes, AND with having swollen my abdominal cavity with as much visceral lard as I could handle: I was due for some subcutaneous fat growth.
By 400 lbs, I noticed significant reduction in the hardness and duration of erections, and also the gathering of a knob of foreskin flesh that hung off the tip of my penis when soft. When I was erect, I found it quite tight on my penis, and it could become uncomfortable.
I also experienced a really blissfull sweet dorsal nerve sensation. Basically if I sat upright and gently rocked back and forth I could feel a divine buzzing through my dick. I’d regularly stretch my gut out whilst doing that, and find that I was dripping precum by the end of it.
I got used to not using my dick for penetration. I was too big and unwieldy to thrust for long, and with the smaller and softer erections, coupled with the awkwardness of a tight foreskin, my sexual activity tended to focus on overeating and masturbation or oral sex.
My opinion was that this form of sex, involving gorging whilst enjoying my tingling, throbbing dick, was far better than regular thrust-climax-cooldown sex.
