Include band, song name, and pretty pretty please a link to the video (to make sure I get the right one in case of variants).
Note however that Tumblr will not allow a link alone, hence the need for all three information.
Submission that fail to include these information may fail to be acknowledged.

No special date request: Things will come out of the queue whenever the time happens to be
Mod may choose to locate a better resolution version at their discretion.
Genre tags will be broad and approximate at best, as I am not particularly good with that.

What counts/can count as a music video
Something filmed or animated specifically to formally accompany a song.
Song remixes, alternative versions, parodies and filk versions are okay, as long as they have official music videos of their own (or as official as can be).
By default, lyrics video are not included for this purposes unless they are a) official and b) prominently involve nontext footage (example).
Segment of concept albums that make up a movie (e.g. the Wall, Interstella 5555...)
Fanmade or animated videos for existing songs may be featured on a case by case basis but only for songs without actual music videos: example.
What doesn't count
Visualizers/Lyrics video with just or mostly text (including kinetic typography videos), unless that is the actual official video.
Musical number clipped as is from their source, with or without added lyrics: example 1, example 2.
Live performances not part of an official music video (official video example 1/example 2)
AMVs, fan animations, montages etc. others than ones potentially qualifying for the "no official video" category: example 1.
