You know those Beauty and the Beast type stories?
Well, I was thinking about something like that, where a nobleman from a very very wealthy is born with a really hideous disfigurement.
It leaves him constantly in pain, with a temperament to match, and no one wants to marry him and be tied down to something so hideous and mean, and forget about having children.
In comes the daughter of a disowned noble person.
Like, there's no title, and vague blood from the past, bribed by a huge payout that would take care of those she cares about.
She goes in with a positive attitude, and a determined heart, knocking down his defenses as she commits to taking care of him, making his life not just easy (Because he has servants to make it 'easy'), but sweeter, from reading or painting with him, to watching sunsets, and helping him see the brighter side in life.
So one day, when she goes to visit him and finds an extraordinarily handsome, fit, able-bodied man with eyes only for her that greets her with a grin, telling her that she broke the curse that was placed upon him with her love, there's just one question,
Little does she realize, that she didn't break any 'curse', but instead he had sold his soul and the souls of those he would sacrifice annually (And just for fun) to keep up this facade for the rest of their shared lives.
Because doesn't his sweetheart deserve a fairytale she could only dream of?

To run with your idea, I'd like to think that "hideous disfigurement" is probably a massive burn covering most of his body that he received from a tragic childhood accident, a genetic illness, or something like that. Obviously, a nobleman who can't attract suitors with their money and looks is cast aside and looked down upon.
I imagine Yandere!Nobleman is no different. Due to his affliction, he doesn't have any friends, his family hardly interacts with him, and of course, no woman is willing to even sit in his presence.
He wasn't sure why you were willing to sit with him, talk with him, or even entertain the idea of seeing him again. What was so different about you, and why weren't you running from him..?
You'd better believe he tried to run you off multiple times. It wasn't that he truly wished for you to leave. He was just so used to being left first, and things like "hope" or "trust" hadn't always been his friends. However, you continued coming back to see him when you could, entertaining him, participating in his hobbies, and being more than a warm body to him.
You were perfect, possibly the only perfect human being to ever exist.
So, what could he do to possibly pay you back for introducing such sweetness into his otherwise bitter life?
The next time you go to visit Yandere!Nobleman, he's a completely different man— literally. His disfigurement is completely gone, he's moving around without an ounce of pain, and his attitude is much more positive! You were beyond confused, even after he claimed that your love and care had broken his curse.
Still, you weren't able to look a gift horse in the mouth. Whatever the cause, you were just happy he wasn't in any more pain—and the two of you lived happily ever after!
Unlike Yandere!Nobleman's victims, of course. Those poor souls, whose only purpose in life was to be sacrificed for his selfish gain. Unfortunately, he wasn't able to escape this "deal" unscathed. His soul was also placed on the chopping block, but he managed to postpone that for as long as you should live. Once you've passed on, he will follow you without a second to spare. This wasn't something you needed to know, obviously. You deserved to be blissfully ignorant to his disgusting deeds. You were going to die together.
Is that not love in its purest form?
