That's shallow women's stuff, an easy target for quips, similar to the jokes on Sam's hair and his salads.
Except. Dr Sexy as a soap opera caters to women specifically, and Dr Sexy is a character written to be a female fantasy. In this kind of media, the audience is made to identify with the women on screen, so they can fantasize about making out sloppy with the sexy doctor in the elevator. And. Dean watches it with such intensity. He follows it, remembers it and blushes over the doctor. Effectively, the show put Dean in the shoes of a woman where it was supposed to be a misogynistic joke, but ended up being genuine
