Fun with Residents

Called the resident because my patient’s BP had dropped dramatically and his HR had risen. 

“Hi Dr. ___, these are the patient’s vitals, a lactic acid hasn’t been collected since admission but he meets several SIRS and sepsis criteria and I am concerned that he is no longer appropriate for this floor. Please call me back”

Resident calls back 

“can we put him on telemetry?”

me: “not on this unit, he will have to be transferred to a dedicated telemetry unit, our policy states we cannot monitor them and our patient ratios are too high to safely watch him the way he needs to be watched without it”

resident: “just give him a bolus and keep me updated”

me: *calls the attending because that doesn’t fly with me*

attending orders an additional bolus and says he will have the resident write the transfer orders to ICU instead of tele

resident calls me: “um yeah so where do I need to send this guy?”

me: “what do you mean? The attending said you were going to transfer him to ICU.”

resident: “yeah but like I don’t know what floor to send him to”

*forever rolling my eyes*
