Probably the most harrowing thing in Challenger: The Final Flight is the revelation in the epilogue title cards that, after everything that came out about NASA and their failure to give the engineers time to fix a problem that they totally fucking knew about, the Columbia disaster in 2003 was caused by a problem that NASA had been warned about but didn't take steps to fix because they were still trying to get launches done on a schedule.
It’s actually sickening.
